Ban appeals your staff can review

The ban DM links to an appeal form, and staff decide in a private channel.

When the bot bans someone, with .ban or through the agent, it first sends them a DM with the server, the reason and an Appeal the ban button. They sign in with Discord on the appeal page, which checks they really are banned, and answer two short questions.

Each appeal arrives in a private #ban-appeals channel that only roles with Ban Members or Administrator can see; Modu creates it when appeals are turned on. Staff press Accept, which lifts the ban, or Deny, and write a reason. The member gets a DM with the decision either way, and a cooldown stops repeated appeals.

.appeal setup turns it all on, .appeal off turns it off, and .appeal shows the link.
